What ends up on the sheet
Legal name, stage name, email, phone, role, PRO and IPI/CAE number, publisher with its own PRO and IPI, and the writer and publisher percentages as two separate pools that each total 100%. That's everything a publisher needs to register the song, which is the part most split sheets leave out.
A split sheet is a binding written agreement between the writers of a song, signed and dated by everyone on it. It's the document publishers and societies rely on when the song is registered, and the one that settles who owns what if anyone later disagrees. Keep it. It does not replace a full songwriter agreement, and it does not register the song for you.
